Why should this person be nominated? (What makes nominee unique; why should they be selected the winner of this category?
April currently is The Director Of Social Services at Catered Manor Skilled Nursing Facility, she is a very important part of the Interdisciplinary Team, where she addresses the psycho-social needs of the residents and discharge planning. When I met April, I immediately knew she was in the health care field because she loves helping people. Her warm demeanor and sincerity shines when you see her interact with her patients. She has the most beautiful eyes as you can see love radiating through them when she speaks. They say that our eyes are the windows to our souls. Anyone who meets April can see that she has a beautiful soul. It's an honor to nominate April Nelson for all her hard work and dedication. She is oh so deserving of this award and most definitely a hero in the eyes of many.

What is this nominee’s involvement in senior care or senior issues
April is truly one of the most compassionate people I've come to know. She is the mother of 5 children and a foster parent to 4, has a heart of gold and a smile that lights up the room. April is a active member in her Church where she teaches in the children church and has spear headed visits to skilled nursing facilities provided encouragement and socialization to seniors. She started in the health care field over 16 years ago as a CNA, she always went above and beyond providing the best quality of individualizes care to her patients. She has a heart of gold and gives so unconditionally to all her patients. She walks the hall smiling, offering help to staff answering call lights, providing supportive conversations and addressing the concerns of both patients and their families, she finds time no matter how busy or difficult the situation. April has a great resources in place to help deal with the most complex cases, she works closely with families to ensure that a safe discharge is in place with the proper referrals and resources are in place to prevent reentry to the hospital , once patients are discharge, Aprils involvement in their care does not stop she places follow up phone calls to ensure all services are in place and answer any questions they may have. She even goes to the extent of providing care services or wellness checks for free for those seniors whom need the care but can not afford it, provides food from her churches food bank. Social Service with a heart as big as Texas.
