List your nominee's experience in Senior Care.
Jennifer Baca has been working in inpatient skilled nursing since 2016. She completed a Masters of Science in Healthcare Administration and Gerontology in 2015. One of her first jobs out of college was at Huntington Terrace Senior Assisted Living facility, where she began as a full-time lead food server. After about a year, she was offered a part-time Activities Assistant position where she assisted the Activities Director in creating activities, hosting classes and spending time with residents from both the Independent Living & Memory Care units. This really implanted her desire and passion to work with seniors in order to make a difference in their lives, as well as their families’. Then she moved on to hold two different positions within Case Management at Fountain Valley Regional Hospital: first as Case Manager Assistant, and then as Managed Care Coordinator. Through these roles, she was able to develop and refine her communication, time management and problem solving skills while working in a fast-paced environment. Furthermore, with her experiences and knowledge gained while working in various health care settings combined with her education, she established a proven ability to problem-solve, implement improvement strategies, study these effects and act to fine tune processes.

Jennifer currently holds the position of Case Manager at Manor Care Health Services, where she oversees all managed care entities in the Case Management & Social Services. Working closely with health plans, medical groups and their case managers and physicians as well as with other facility departments, she has implemented processes that have improved patient care and satisfied providers. She has and continues to work to build relationships with members of administration and management within her organization, as well as with ancillary and community organizations to ensure that focus is always on the best interest of the patient and their family.
